| Summary: | As part of the research project "Documentation of clinical pastoral care in the horizon of interprofessional spiritual care" of the Swiss National Science Foundation, a documentation tool was developed in the palliative care department of the Cantonal Hospital of Graubünden, which is intended to improve interprofessional cooperation between pastoral care and health professionals for the benefit of good patient care. The article describes the development process and the conceptual basis for the documentation tool and discusses it in the context of international developments. The tool is presented and the article concludes with reflections on the perspective of chaplaincy documentation practice. |
